and ready. Personal protective equipment requirements are also clearly
identified.

continuously monitor the work area during the task and for a defined period
afterward, often between 30 and 60 minutes or as required by site rules. The
permit should name responsible supervisors and issuing authorities, define when
revalidation is necessary, and specify start and end times. Proper close-out
procedures ensure the permit remains controlled from beginning to end.
Incidents linked to hot work rarely occur because risks were
unknown; they usually result from poor execution. Common failures include
inadequate housekeeping, abandoned fire-watch duties, incomplete isolations,
undocumented changes in work scope, and weak recordkeeping. These issues can be
prevented with structured checklists, disciplined permit handovers, clear
triggers for revalidation, and mandatory post-work checks.

demonstrates its value.
